Family Child Care Connections Enrollment

 To enroll in the Family Child Care Connections Program please email Ms. Christina Gonzales gonzalesc@childcarecenterofevanston.org. 

You will be given the name, address and phone number of one of our Family Child Care Connections providers that has an opening. You will contact them directly for a tour of their program. If you feel that this home would be a good fit for your family you will call Ms. Christina at 847-864-5610 to arrange for a time to come in to the Center to complete paperwork before your child’s start date. Family Child Care Connections Handbook

 DCFS licensing also requires us to have a current (within six months)  Illinois Physical Exam Form IL444-4737_Physical_Exam_form.pdf (59 KB) completed. You may download this form and ask your pediatrician to complete it and fax it back to us at 847-869-2687. Please be sure to fill out the parent health history portion of the form. We will also need a copy of your child’s birth certificate.

Family Child Care Connections Tuition Rates for

September 1 to August 15, 2012

Tuition:  Full-time

          Infant (six weeks to one year) = $285.00 per week
                       Toddler (one year to five) = $260.00 per week 

Tuition:  Part-time 

         Infant (six weeks to one year) = $62.00 per day
    Toddler (one year to five) = $57.00 per day

 (Part time days only – not mornings or afternoons)
