Process

Family Child Care Connections of Evanston maintains a waiting
list.

We advise parents that are expecting or have very young children
to place their name on many waiting lists as soon as possible.
There is less availability of infant slots compared with slots
for children 10 months and older.

When Family Child Care Connections of Evanston has one or sometimes
more than one opening within the network a family is contacted
by the Program Director.

The family is given information about the current openings;
location, date of opening and Family Child Care Connections Provider's individual
information.

Families are then encouraged to call the Family Child Care Connections Provider
to schedule a time to observe their program and interview with
them in their home.

When the family chooses a Family Child Care Connections Provider's program
that is the right fit for them, they contact the Program Director
to schedule a time to complete the intake process.

Required Documentation
The following documentation is needed for the Intake Process.

Child's current Illinois Department of Public Health Certificate
of Infant and Toddler Health Examination (http://www.state.il.us/dcfs/docs/cfs600i.pdf)

Contact information for people listed as emergency contacts

Deposit - One week's tuition, which will be applied the
first week of childcare. (Non-refundable
if the child does start on the agreed upon date of enrollment.)

Families subsidized by Action for Children will also need:

To be working a minimum of 25 hours per week during
childcare hours

And/or be attending school or a training program that
is accepted by Action for Children

Two current and consecutive pay stubs and if applicable,
school or training documentation.
